Service-Matching and Response Tool (SMaRT)

SMaRT is a decision-support tool aimed at providing a full-set of options to consider at any stage in an individual consumer’s recovery.

Your responses below are based on your assessment of the consumer’s current presentation. Once you have selected “Yes” to the relevant Matching Presentations (1) and chosen the current Phase of Care (2), click the “Calculate” button (3) to view a ranked list of service options.

Your selection of services for the consumer are always grounded in clinical decision making and you are not bound by the ranking provided by this tool in making decisions.
